Click "option" and check "allow this content".

If you have layered forms

You might get a message shown below when printing out.

Solutions

Change the position of check boxes so that they won't be layered. Or decrease the number of paragraphs and make more space between lines.

If you can't place the check boxes neatly

Select "Grid" in "Adjust objects" in Word to make less space between grid lines.
